Request for suspension of act
You can send your application in one of the following ways:
- by post in paper form to the address of the Office, signed by the applicant's handwritten signature,
- orally, in the minutes of the Office in the Registry (during office hours),
- electronically, with a qualified electronic signature of the applicant, i.e. via the Central Portal of Public Administration slovensko.sk.
An employee-whistleblower's request to suspend an employment action shall include:
- the name, surname, date of birth and residence of the whistleblower,
- the name of the whistleblower's employer,
- an indication of the employment action that the whistleblower is requesting to suspend (for example, termination of employment),
- the date on which the whistleblower became aware of the employment action (for example, the date of delivery of the notice),
- the date on which the whistleblower made the report,
- the name of the authority to which the whistleblower made the report,
- a justification as to why the whistleblower believes it is retaliation for making the report,
- attachments (employment contract, notice of termination of employment, copy of notification of anti-social activities).
